Individuals who have medical necessities and need care after hospitalization and are homebound. Anyone can make a request for a home health evaluation. All nursing and therapy services are provided under Physicians Care.

How can I receive Home Health Services?

Anyone can make a request for home health care as long as it is validated by their physician.  If a patient feels as if they need home health care, it is ultimately their choice as to which agency they will go with.  If you or someone you know has had a good experience with an agency, be sure to tell your physician or discharge planner of the hospital who you would like to take care of you. The criteria to receive home health care is that you must be home bound and have a condition that requires a need for skilled nursing or therapy.
